Factory Questionnaire to assess social, ethical and environmental aspects

We run a clothing company called Silverstick - www.silverstick.co.uk

 

Would anyone have or know where we could get our hands on a questionnaire for factories that covers ethical, social and environmental aspects? We are after a concise document that we could use to vet factories.

 

Any pointers much appreciated.
